This dish reminds me of the Taco Bell Nacho Supreme, but the original version included beans, followed by meat, nacho cheese, tomatoes (not salsa), and sour cream. It did not have rice in it. Back in the 90s, I was a training manager at Taco Bell and still remember how to make many of their menu items.
